~ A Final Farewell ~

It has been my profound joy to have cared for all of you over the past forty-five years. Thank you, so much, for providing me that opportunity. I will miss you all. God bless.

Dr. David Hartzok

~ Records ~

The plan for Dr. Joel McGahen of Chambersburg to be the legal custodian

of your optometric records never materialized.

It was always my hope that your ocular history would be easily accessible

to a capable practitioner willing to facilitate your ongoing care. Many of you have

lengthy and somewhat complicated histories that require careful follow-up.

Thankfully... Dr. Sandra Fink-Freeman, an independent optometric practitioner

in Greencastle, has agreed to continue your care. She and her associate, Dr. Nayrobi

Cepeda, will provide comprehensive eye examinations and full optical services.

After February 12, Dr. Fink-Freeman will be in possession of your records

and will handle all future patient requests and inquiries.

Office location:

Greencastle Eye Care Center

50 Eastern Avenue, Suite 107

(John L. Grove Medical Center)

Greencastle, PA  17225

717-597-7708

It was my sincere intention for my patients to be spared any inconvenience in the

continuance of their future eye care needs. I apologize that the original plan failed.
